A completed Player Profile is required for participation in all programs at The Pitch. Profiles must be updated annually to ensure information remains accurate.
THE PITCH LLC
RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT
In consideration of being allowed to use the facilities, services, and programs of The Pitch LLC, located at 412 S. Kit Avenue, Caldwell, Idaho (hereinafter “The Pitch”), the undersigned participant (or parent/legal guardian if under 18) agrees to the following terms:
1. Assumption of Risk
I understand and acknowledge that participation in indoor soccer and related activities at The Pitch involves inherent risks, including but not limited to: slips and falls, collision with other players or equipment, sprains, fractures, concussions, permanent injury, illness, or even death.
These risks may result from my own actions, the actions of others, the condition of the facilities, or the negligence of any party.
I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation.
2. Waiver, Release, and Hold Harmless Agreement
To the fullest extent permitted by law, I hereby:
Waive and release any and all claims, liabilities, demands, actions, and causes of action against The Pitch LLC, its owners, managers, employees, agents, insurers, landlords, representatives, successors, and affiliates (collectively, the Released Parties”) arising out of or related to any injury, illness, disability, death, property damage, theft, or other loss that I may suffer while using or being present at the facility, whether such claims arise from negligence, premises liability, the actions or omissions of third parties, or otherwise.
Agree to hold harmless and indemnify the Released Parties from and against any and all claims, suits, judgments, damages, losses, costs, or expenses (including attorney’s fees and court costs) brought by or on behalf of myself, my child(ren), or any third party arising out of or in any way connected with my (or my child’s) participation in activities at or associated with The Pitch, including but not limited to participation in leagues, practices, training sessions, or events.
This Waiver, Release, and Hold Harmless Agreement is intended to be broad and allencompassing, covering any and all potential legal claims, and applies regardless of whether the injury or loss is alleged to have been caused in whole or in part by the Released Parties.
3. Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, suits, damages, costs, losses, and expenses (including attorney’s fees) arising from or related to my participation in any activity at The Pitch, including but not limited to claims
brought by third parties or co-participants.
4. Medical Authorization
In the event of an injury or illness, I authorize The Pitch to seek emergency medical care for me (or my child), and I accept responsibility for any medical expenses incurred. I acknowledge that The Pitch is not obligated to provide medical care and may not have staff trained in medical assistance.
5. COVID-19 and Communicable Diseases
I understand that despite all reasonable precautions, The Pitch cannot guarantee the absence of communicable diseases such as COVID-19. By entering the premises and participating in activities, I voluntarily assume all risks related to exposure or infection and waive any and all claims against the Released Parties in this regard.
6. **Severability and Governing Law**
If any portion of this agreement is found to be invalid or unenforceable, the remaining sections shall remain in full force and effect. This agreement shall be governed by the laws of the State of Idaho, and any legal action shall be brought in Canyon County, Idaho.
7. Acknowledgment of Understanding
By checking the acknowledgment box during registration, I confirm that I have read, understood, and voluntarily agree to all terms outlined in this Release of Liability, Waiver of
Claims, Assumption of Risk, and Indemnity Agreement. I understand that checking the box serves as my legally binding signature.
If signing on behalf of a minor: I certify that I am the parent or legal guardian of the participant and have the legal authority to sign this release on their behalf.
8. Minor Participation in Adult Leagues and Activities
I understand and acknowledge that minors aged 17 or older may be permitted to participate in leagues, games, or activities involving adult participants. I understand that participation with adult participants may involve increase risks. I expressly consent to the minors participation in such activities and voluntarily assume all associated risks on their behalf. I agree that The Pitch LLC shall not be liable for any injuries, damages, or losses arising from the minor’s participation in activities involving adult participants.
The Pitch Code of Conduct
At The Pitch, we believe soccer is more than just a game — it’s a community. Every player, coach, spectator, and staff member plays a role in shaping the culture of our facility.
By stepping onto the field, you are committing to uphold the following standards:
Lead with Integrity
Compete with honesty and fairness. Respect the game, your opponents, your teammates, and yourself.
Show Respect
Treat all players, referees, staff, and spectators with courtesy. Discrimination, harassment, bullying, or threatening behavior of any kind will not be tolerated.
Play with Spirit
We love passionate play — bring your energy. But play hard and fair. Good sportsmanship is expected at all times.
Set the Standard
Every adult at The Pitch helps set the tone for the next generation of players. Lead by example — your actions today help build a better soccer community tomorrow. The youth may be watching!
Support Youth Soccer
When you participate in adult leagues at The Pitch, you help fund programs that we offer, and help keep our doors open which makes soccer more accessible for kids in our community. Thank you for being part of something bigger.
Zero Tolerance Policy
The Pitch has a strict zero tolerance policy for:
- Fighting or physical altercations
- Abusive, threatening, or discriminatory language
- Aggressive or intimidating behavior toward referees, staff, or players
- Intentional dangerous play or retaliation
- Vandalism or property damage
Any violation may result in immediate removal from the facility, suspension, or permanent ban without refund.
